Fees and Insurance

I have a fee-for-service practice. The majority of my patients have ‘Out-of Network Plans’ with their Insuror. If you have such a plan I will provide a ‘Super Bill’ when requested for you to submit. Treatment fees will always be discussed during a telephone conversation prior to the Initial Consultation.

I am a provider for the Medicare ‘Standard’ plan. I am not on any of the Medicare Advantage panels..
